- This website is operated by the certified NPO Association for Supporting Cerebrospinal Fluid Hypovolemia Patients (hereafter referred to as “ ASCFHP”); therefore, the donation will be paid to ASCFHP. The donator (hereafter referred to as “Donator”) shall acknowledge said fact.
- Donator may donate up to $4,280 at one time to ASCFHP using the credit card payment website and may use the website up to three times per day.
- Once a donation is made by credit card payment, no refund shall be made for any reason.
- The billing name to be sent to Donator from each credit card company will be “csfjapan 8145 716 4646.”
- This credit card payment shall be settled through credit card settlement agent company Another Lane, Inc.
- The postal code, address, telephone number, etc., to be filled in on the credit card application form may be filled in on a voluntary basis
- If you make a donation in your country to an NPO in another country, responses can be made as necessary such as if tax deduction is applicable or if you require
- a certificate. Our association is a certified NPO that is subject to tax deduction in Japan.
